Tooth gem warranty replacement **with receipt**, within 3 months from original placement. After 3 months schedule for new tooth gem. No call/No Show cancellations will result in a forfeit of the warranty.

To cancel or reschedule please contact the office one hour before appointment or deposit will be forfeited. All deposits are non refundable.
